Triad Behavioral Resources (TBR) is a fully virtual MAT service provider. Patients need a phone or other internet-enabled device with Zoom downloaded, and an internet connection to receive care. The only time a patient needs transportation is to get to their preferred pharmacy once a week to pick up their medications. We are a low-barrier program that supports patients wanting to work, manage complex and busy life schedules, and return to a sense of normalcy in life as productive members of society. We also work with independent re-entry support groups like Our Journey 2Gether, as well a providing support for DSS and Probation workers and their clients who are in treatment.Medication Assisted Treatment Assessments Counseling Group Urine Drug Screens (filled at home, overnighted to the lab, DNA authentication available) Peer Support (we would love to speak with someone about increasing this service offering, as a virtual service provider, we do not meet the requirements to bill for this service since only 20 percent can be provided virtually)6626 Gordon Rd STE CNew Hanover

LINC provides a range of low-barrier options for individuals to access services, reflecting our commitment to meeting people where they are. Our approach is designed to eliminate unnecessary obstacles to care and support, recognizing that each individual’s path to stability and wellness is unique. Individuals can access care via walk-in services that function much like a resource center without walls, allowing individuals to receive immediate assistance without the need for appointments or referrals. In addition to walk-ins, individuals can access services directly by reaching out to LINC themselves via telephone, email, and/or text and/or through a variety of other channels such as family members, friends, or members of a person’s social network are welcome to inquire on their behalf, and prison case managers or other professionals involved in an individual's care and reentry planning may also initiate contact. This flexible and inclusive access model ensures that people can connect to housing support, case management, behavioral health services, workforce development, and other critical resources in a way that best fits their needs and circumstances.LINC offers a comprehensive range of services designed to support individuals and families impacted by incarceration through a holistic and person-centered approach. Housing is a cornerstone of LINC’s programming, with options including transitional housing at the M.E.R. Residential Living Campus, which provides a structured, sober environment with meals, clothing, and life skills training, as well as permanent supportive housing opportunities and affordable housing. LINC also offers employment and workforce development through low-barrier employment bootcamps, exploration workshops, and vocational training in fields like hospitality, culinary arts, and commercial driving. LINC also provides intensive case management and reentry support through the Local Reentry Council, helping individuals navigate housing, employment, education, behavioral health, substance use treatment, and access to benefits. Mental and behavioral health services include cognitive behavioral interventions (CBI), group therapy, dual-diagnosis support, and referrals to appropriate treatment providers. Beyond direct service, LINC facilitates community integration through initiatives such as the Healthy Opportunities Pilot, which connects participants to coordinated care that addresses physical and behavioral health as well as social needs. Peer mentoring, support groups, and community engagement efforts round out a robust system of care designed to reduce recidivism, build resilience, and empower individuals to thrive. All services are accessible through various pathways—including self-referral, walk-ins, inquiries from family or support networks, or referrals from correctional and community-based partners—ensuring flexible and inclusive access to care.We have several offices in New Hanover County. Our mailing address is Post Office Box 401, Wilmington, NC 28402. These include: Referral from Community Partners or Justice System Entities Individuals may be referred by probation/parole officers, jail-based case managers, reentry coordinators, court systems, or law enforcement partners. Managed Care Organization (MCO) Navigation Members enrolled in Trillium Medicaid Direct or the Tailored Plan can contact Trillium’s Member & Recipient Services to receive help identifying providers, scheduling appointments, or connecting with care coordination. People access care through various means, depending on their needs and circumstances. Here are some common ways: 1. **Primary Care Physicians**: Many individuals have a primary care provider who they consult for general health issues, routine check-ups, and referrals to specialists. 2. **Health Insurance**: Most people access care through health insurance plans, which cover a range of medical services. They may obtain insurance through their employer, government programs, or purchase it privately. 3. **Urgent Care Centers**: For non-emergency situations that require immediate attention, people often visit urgent care facilities. These centers provide treatment for minor injuries and illnesses without the long wait times typically associated with emergency rooms. 4. **Emergency Services**: In case of severe or life-threatening conditions, individuals can access care through emergency services by calling 911 or going directly to a hospital's emergency department. 5. **Specialists**: For specific health concerns, patients may need to see specialists, such as cardiologists, orthopedists, or dermatologists. Referrals from primary care providers are common for these visits. 6. **Telehealth Services**: With advancements in technology, many people now access care remotely through telehealth services. This allows patients to consult with healthcare providers via phone or video calls. 7. **Community Health Centers**: These centers provide comprehensive healthcare services, often focusing on underserved populations. They may offer services on a sliding fee scale based on income. 8. **Pharmacies**: Many pharmacies now offer health services, including flu shots, health screenings, and consultations for minor health issues. 9. **Online Resources**: Some individuals turn to online platforms for health advice, symptom checkers, or to book appointments with healthcare providers. 10. **Family and Friends**: People often rely on recommendations from family and friends to choose healthcare providers or facilities, especially if they are new to an area. Accessing care can also vary based on factors like geography, socioeconomic status, and healthcare policies in different regionsWe provide substance use, behavioral health, reentry support, life skills support, diversion support, recidivism reduction, mentoring program, forensic evaluations.
