These include: Referral from Community Partners or Justice System Entities Individuals may be referred by probation/parole officers, jail-based case managers, reentry coordinators, court systems, or law enforcement partners. Managed Care Organization (MCO) Navigation Members enrolled in Trillium Medicaid Direct or the Tailored Plan can contact Trillium’s Member & Recipient Services to receive help identifying providers, scheduling appointments, or connecting with care coordination. Self-Referral Eligible individuals or their family members can directly contact network providers to schedule an intake or request services. Providers often assist in verifying Medicaid coverage and coordinating care entry. Transition and Reentry Planning For justice-involved individuals, access may be facilitated through coordinated discharge planning, where care teams collaborate with correctional facilities to establish post-release support. Provider Outreach and Community Events Providers may also engage individuals through community outreach, education, and walk-in access points to promote service availability. Emergency or Crisis Services In acute situations, access may begin via emergency departments, mobile crisis teams, or crisis stabilization units, with follow-up care coordinated through the MCO or provider network.Harper & Associates is a community-based behavioral health agency committed to delivering person-centered, trauma-informed care across a continuum of support settings. We serve individuals with Severe and Persistent Mental Illness (SPMI), Intellectual and Developmental Disabilities (I/DD), and co-occurring behavioral health needs. Our services include: Community Behavioral Health Services Outpatient Therapy & Clinical Services: Individual, group, and family therapy using evidence-based modalities including CBT, DBT, and trauma-focused approaches. Justice-involved youth and their families access care through multiple coordinated, trauma-informed pathways designed to reduce barriers, ensure family engagement, and support early intervention: Referral and Intake Process Referral Sources: Referrals are accepted from Managed Care Organizations (MCOs), juvenile court counselors, diversion programs, detention centers, schools, Department of Social Services (DSS), residential treatment providers, primary care physicians, and directly from parents, guardians, or caregivers through self-referral. Referral Submission: Referrals can be submitted via our secure online form, direct phone call, email, or fax. Our Client Access and Support Team (CAST) promptly triages all youth referrals and provides a response within 1–2 business days to coordinate next steps with the family and referral source. Verification & Eligibility: Intake specialists verify insurance coverage, legal custody status, identification, and any court orders or parental/guardian consent forms. Support is provided to families in gathering any required documents, including those related to guardianship or protective services. Assessment & Intake Initial Clinical Assessment: A licensed clinician completes a Comprehensive Clinical Assessment (CCA) within 7 days of intake, considering developmental stage, trauma history, educational and social functioning, and involvement with the juvenile justice or child welfare system. Service Matching: Youth are matched to appropriate services based on diagnosis, age, functional needs, legal status, family dynamics, and regional availability of providers and support teams. Family involvement is prioritized throughout the process. Access Modalities In-Office Services: Youth and families may receive services at any of our conveniently located outpatient offices across North Carolina and Georgia, with expansion planned in South Carolina and Virginia. Telehealth Services: Secure, HIPAA-compliant virtual services are available for therapy, case management, care coordination, family sessions, and peer support, based on the youth’s clinical appropriateness and family preference. Community-Based Services: We provide services in schools, homes, shelters, community centers, group homes, residential programs, and juvenile detention facilities. When applicable, we coordinate with court diversion programs, reentry supports, foster care agencies, and Wraparound teams to promote continuity of care.Our agency offers a comprehensive, culturally responsive, and trauma-informed continuum of care for individuals with behavioral health needs, with specialized programming for justice-involved adults: Behavioral Health Services Outpatient Therapy (OPT): Individual, group, and family counseling focused on trauma recovery, anger management, emotional regulation, reintegration, and support for co-occurring disorders. Substance Use Counseling: Evidence-based treatment for individuals with substance use disorders and co-occurring behavioral health conditions. Behavioral Health Assessments: Diagnostic evaluations, CCAs, and reports tailored for treatment planning and legal proceedings. Peer Support Services Certified Peer Support Specialists with lived experience offer mentoring, wellness coaching, life skills training, and advocacy. Peer support is integrated into treatment teams to enhance hope, motivation, and continuity of care—particularly valuable during transitions out of correctional settings. Research-Based Behavioral Health Treatment (RB-BHT) Specialized behavioral therapy for individuals with Autism Spectrum Disorder (ASD) or Intellectual/Developmental Disabilities (IDD), especially those involved with the justice system or experiencing behavioral crises. Case Management & Care Coordination Care Coordination: Assists clients with housing, transportation, employment readiness, benefits access, and legal advocacy. Reentry and Transition Planning: Focused support for individuals transitioning from incarceration to community-based services or alternative diversion programs. Specialized Consultative Services (SCS) Behavioral consultation services for individuals with IDD and complex support needs, often involving coordination with interdisciplinary teams (IDT). Community-Based and Life Skills Services Community Living and Support (CL&S): Skill development for independent living, safety, and daily functioning. Supported Employment: Vocational support for justice-involved individuals with behavioral health or IDD diagnoses seeking job readiness or reintegration into the workforce.
